Has the applicant or stated person been convicted of any crime or misdemeanor?

** PURSUANT TO THE VILLAGE OF CLEVES, OHIO CODE OF ORDINANCES, SECTION 111, AND APPLICABLE OHIO LAW, ALL APPLICANTS FOR A VENDOR/SOLICITOR PERMIT SHALL BE SUBJECT TO A CRIMINAL HISTORY BACKGROUND CHECK CONDUCTED BY OR ON BEHALF OF THE VILLAVE OF CLEVES. THE PURPOSE OF THIS BACKGROUND CHECK IS TO VERIFY COMPLIANCE WITH DISQUALIFYING CRITERIA SET FORTH IN THE CODE OF ORDINANCES, WHICH PROHIBIT ISSUANCE OF A PERMIT TO ANY PERSON CONVICTED OF CERTAIN CRIMINAL OFFENSES **

If the applicant proposes to handle foodstuffs, please attach a statement from a licensed physician, dated no more than 14 days prior to the date of the application, certifying the applicant free of contagious or communicable disease.

Submission of this application electronically does not constitute approval. The application will not be processed or considered complete until:

  • The $40 application fee is paid in full to the Village of Cleves during normal office hours.

  • All required supporting documentation, including a clear headshot, any necessary food-handling health statement, and any required business credentials, are received.

  • A criminal history background check, as required under the Village of Cleves Code of Ordinances §111, is completed and the applicant is confirmed to meet eligibility requirements.

Issuance of a license is contingent upon successful completion of these requirements and compliance with all applicable provisions of the Village of Cleves Code of Ordinances. By submitting this form, the applicant affirms that all information provided is true and accurate to the best of their knowledge, understands that knowingly providing false or misleading information may result in denial or revocation, and acknowledges their responsibility to comply with all applicable laws and regulations.

I AUTHORIZE THE RELEASE OF ANY AND ALL INFORMATION TO THE VILLAGE OF CLEVES

Solicitor Agreement

By signing below, the undersigned hereby certifies their understanding of the below vendor and solicitor agreement and agrees to abide by all applicable rules, regulations, and policies regarding such license. The undersigned also acknowledges that it is unlawful to knowingly provide false information in the license application, and hereby agrees that the information stated in the application is, to the best of their knowledge, true and correct.

 

The undersigned understands that any change in information required by division (A) of the Village of Cleves Ohio Code of Ordinances §110.02, must be reported to the Clerk or other authorized official within 14 days of the change. The undersigned understands that the license shall not be valid beyond the expiration date therein specified (90 days after the date of issuance), and that every license is not transferable.

 

By signing below, the undersigned acknowledges and agrees to the penalties for violation of any general provisions within Title XI of the Village of Cleves Ohio Code of Ordinances. It is acknowledged that anyone who violates any provision of this title, for which another penalty is not already provided, shall be fined no more than $500, imprisoned for not more than six months, or both. The undersigned also understands that their license may be suspended or revoked for any of the reasons stated in §110.07 and §111.06, as well as their ability to repeal the denial, suspension, or revocation of the license.

Those who sign below are aware of the fact that any license issued to an itinerant merchant shall be posted conspicuously in or at the place named therein. If the licensee is a peddler or solicitor, the license shall be kept with the licensee during such time as he or she is engaged in the business. Failure to comply is in violation of Title XI.

SOLICITATION POLICY AND NOTICES
The undersigned acknowledges that soliciting outside of the hours of 9:00 am and 9:00 pm are prohibited, and doing so is in violation of Title XI.

 

The undersigned agrees that residents shall make the determination of whether solicitors shall be, or shall not be, invited to their respective residences according to §111.09 of the Village of Cleves Ohio Code of Ordinances. Solicitors will be made aware of which residents do not invite soliciting on their property by observation of a weatherproof card exhibited on the main entrance door of the residence.

 

The undersigned agrees to leave the premises peacefully if the occupant asks them to. The undersigned also agrees to not create any sound or display in any manner with the intent to attract the attention of the occupant of the home when the weatherproof card stating that solicitors are not invited to the residence is on display. Furthermore, if the resident is stated in any manner to the undersigned to not allow soliciting, even without a weatherproof card, the undersigned will not try to gain the attention of these residents. Failure to comply is in violation of Title XI.

 

The undersigned agrees to pay the $40 application fee for licensing of an individual peddler or an individual solicitor as defined in §111.01 of the Village of Cleves Ohio Code of Ordinance as authorized under §111.02.
